Referral Templates for Referring Practitioners


Simply click on the relevant referral form, complete it for your patient and fax it to (08) 9390 9327.  
​We now have two head office locations.

The Chronic Disease Management Care Plan (Formerly Enhanced Primary Care), is a referral form that needs to be completed by your GP or a care plan nurse.  When you book an appointment you may need to let the receptionist know that you are in need of a CDM referral as the appointment you need to book may require a longer consultation.  CDM refers to chronic medical conditions that may be covered by the Medicare scheme that have been present, or are likely to be present, for six months or more (this is taken directly from the Medicare site).

Type 2 Diabetes Group Sessions  -  As outlined by Medicare; patients who have type 2 diabetes are eligible for one individual assessment session and up to 8 group sessions per calendar year.  Note: these services are in addition to the five individual services for patients with a chronic or terminal medical condition and complex care needs

DVA  - Department of Veteran Affairs offer support to DVA patients by providing free services for Allied Health.  Your referring practitioner must fill out the form which is accessible by clicking on the highlighted DVA at the beginning of this paragraph.

​Workers Compensation - We offer competitive rates for Workers Compensation services whilst still maintain a high standard of service delivery.
